How to Apply

Applicant Requirements for Assistance

In order to be eligible for financial assistance from The Northwest Cancer Centers Foundation, you must:

• Be in active treatment or within 3 months of receiving treatment for an Oncology diagnosis in Lake, La Porte or Porter Counties, Indiana
• Complete the Assistance Application Form
• If requesting utility assistance, you must provide a bill or documentation for the requested assistance

The Northwest Cancer Centers Foundation can assist with the following expenses:

• Utility bills (including electric, phone for 1 land or cell phone, basic internet)
• Child care or Elder care to a licensed provider with documentation
• Groceries
• Transportation to and from appointments via cab, ride share service, or gas station gift cards

We ARE NOT able to assist with the following expenses:

• Rent & Mortgage Payments
• Health, Life, or Home Insurance
• Entertainment Service Bills (Cable, Direct TV, Streaming Services, etc...)
• Any services or reimbursement to providers at The Northwest Cancer Centers, other medical providers or pharmacies.
